On or about October 25, 2021, there was an external data security breach of our systems. A breach of our email system occurred through the email account of one employee. Information which may have been accessed includes name, date of birth, social security number, address, medical record information including psychiatric and physical diagnoses and treatment, and financial information (including insurance). We have attempted to notify via mail those individuals whose data may have breached.
Because we have not identified the individuals responsible or retrieved the data, we recommend you be vigilant and take appropriate steps to protect yourself. Specifically, we recommend that you place a fraud alert on your credit file. A fraud alert tells creditors to contact you before they open any new accounts or change existing accounts. You may also want to contact the applicable bank or other financial institution. Information about credit reporting, fraud alerts and other items relating to identity theft can be accessed at www.usa.gov.
Even if you do not find any suspicious activity on the initial credit reports or relating to any accounts, the Federal Trade Commission (FTC) recommends that you continue to check your credit reports periodically. Victim information is sometimes held for use or shared among a group of thieves at different times. Checking the credit reports and accounts periodically can help you spot problems and address them quickly.
If you find suspicious activity on the applicable credit reports or have reason to believe the information is being misused, you are encouraged to contact law enforcement officials and file a report. You should also get a copy of the report since many creditors will request the information it contains. We also encourage you to visit the FTC website at www.consumer.ftc.gov for additional information from the FTC to help you guard against and deal with identity theft.
